Yartzeit Memorial
Would you like to memorialize a loved one by hanging a glass tile on Mishkan Shalom's Yahrzeit Memorial? We welcome memorial names from members and friends of Mishkan Shalom.The memorial is in Mishkan's main stairwell, flooded with light from the third-floor atrium.
To make a request for a tile:
- Read the Yahrzeit Memorial rules and guidelines (pdf), which includes a full description of the Yahrzeit Memorial, a photo of sample tiles, cost and a donor form.
- Complete the form and send it as an attachment to yahrzeitmemorial@mishkan.org or print and complete the form by hand (please print clearly) and mail or deliver it to the office. (One form per tile.)
- Make your payment online. You may also pay by check. Please make your check payable to "Mishkan Shalom," note "Yahrzeit Memorial Tile" in the memo line and deliver it to the office.
